Event/Meeting Cancellation Request

Please use this form to request the cancellation of an event or ministry meeting and its removal from the church calendar. Submissions should be completed by an authorized ministry leader or event organizer and will be reviewed by the appropriate church staff to update the calendar. 

(i.e. Media Ministry, Smith Wedding, Church Anniversary.)

This request will be submitted for normal review and is subject to approval by church leadership. This request assumes that all other information from your original request remains the same (time, room requested, etc.) If any of the aforementioned items have changed, submit a new event or meeting request form, upon completing this form.
